Information Collected

When you book an appointment I collect personal details such as your name, email and telephone number. This information is used to identify you and contact you about your appointments.

To provide a safe and effective massage or reflexology treatment I also need to collect information about your current health and medical history. This information is only used to make sure your treatment is as effective as possible, and to identify any possible contra-indications to treatment.

Consent

By booking an appointment and filling out an initial or repeat client consultation form, you are consenting to the use of your information as above.

It will never be used for any secondary purpose, such as marketing, without your consent, and it will never be disclosed to any third party without your consent, unless I am required by law to do so.

You can withdraw your consent at any time by emailing bridgnorthholistictherapies@gmail.com.

I am a member of the Federation of Holistic Therapists (FHT) and hold appropriate professional insurance through the FHT. I am required to keep client records for a period of ten years after the final appointment, after which time they will be deleted. If you wish to withdraw your consent during this timeframe, your records will be archived and deleted after this ten year period has passed.

You have the right to review the personal information stored about you at any time, and you also have the right to request to update or amend your data if it is incorrect.

Data Storage

Information is collected via an online consultation form on a platform called Jotform. All submitted information is stored on secure European servers, compliant with UK/EU data laws.

Complaints

If, for any reason, you are unhappy with how your data is handled, please raise your concerns so we can seek a resolution. If you are still not satisfied, you have the right to complain to the Information Commissioner’s Office (ICO).
